Mosaic is looking for aLicensed Practical Nurse (LPN) to join our team!In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.

What YOU'LL Coordinate physician and health care services for people served Conduct health care assessments and write health care plans Facility training and orientation of new staff regarding healthcare procedures Provide on call assistance to staff as needed SCHEDULE:
Nurse #1:
Four 10 hour shifts a week 6a-4pm.
This would include every other weekend as well and rotating holidays.
Nurse #2:
Four 10 hour shifts a week 8a-6p.
This would include every other weekend as well and rotating holidays.
GRAND Nurse #1:
Four 10 hour shifts a week Noon-10pm OR Five 8 hour shifts a week 2pm-10pm including e/o weekend and rotating holidays.
Recommended Skills Assessments Medical Records Team Working Time Management Receptive Health Care Estimated Salary: $20 to $28 per hour based on qualifications.
